Thanks everyone for the help!

Along with the advice above I was told in a private email that, 
"dp_isr = 0x43 means that the ethernet controller has pending 
interrupts. Either the ethernet cards generates an interrupt 
other than 3, or the motherboard does not deliver IRQ 3"

I chagned the IRQ from 3 to 10 and it worked.

Now the F5 stats are as follows:

dp8390 statistics of port 0:
  ...I've got numbers...
  dp_isr=0x0+0x0, de_flags=0x310
dp8390 port 1 is disabled
