Let me answer this before Kyle starts talking about failsafes and
Grissom.  :-)

XEmacs 19.14 was delivered with mailcrypt .elcs that were out of date
and would bomb when used.

Mailcrypt is dependent on Gnus for some macros.  Just prior to
19.14 release Gnus was updated in a last-minute quick fix and the
macros that mailcrypt depended on were changed.  This would have
been O.K. if all the elisp were rebytecompiled, or if Chuck had
understood the dependency and rebytecompiled mailcrypt.  However he
didn't (Gnus 5 was added at the last moment too), and the person who
was probably in the best position to test and find this problem (it
was documented in the Gnus FAQ at the time) was doing his final
last-minute testing unknowingly with a private copy of mailcrypt
which *had been* bytecompiled against the distribution Gnus.

It was a total breakdown in quality control and testing procedures,
and I'm still angry at myself about it (and ashamed because I tested
explicitly for the problem that showed up, and blew it).
