Realism isn't a particularly worthwhile goal; I would recommend
against doing this; or to misleading quote Stuart Allen:

When this subject came up several years ago, it was suggested
that this sort of thing might be cool so long as it all happened
automatically, objects being transferred to your backpack when
you didn't need them, and objects being brought forth when you
did need them. Otherwise you end up with people typing
"GET APPLE WITH LEFT HAND", which is in practice no more realistic
than demanding people move from room to room by saying "RAISE
LEFT FOOT. SWING LEFT LEG FORWARD. LOWER LEFT FOOT. SHIFT WEIGHT
ABOVE LEFT FOOT. RAISE RIGHT FOOT."

I implemented a variation on the "objects automaticlaly transferred
in and out of your backpack" in Heroes for the Thief character, but
it didn't turn out to get used often enough to make much difference.
