FAQ's can be found in this newsgroup or on some minix archive (ftp.nic.funet.fi
still has a minix tree, I think) and the book is Tananbaums book 
'operating systems design and implementation' which describes Minix version
1.[3|5] (I'm not sure which version of source code is listed in the book).
There is new book being written, but I do not know any details about that...
